Member Safety Initiative: Medical Information Form Program
The safety and well‑being of our members is always our top priority. In a medical emergency it is important for responders to have quick access to accurate information — especially since most of us play without spouses or family members who know our medical history.
To help support faster, safer care in the event of an emergency, WKPC is introducing a voluntary Medical Information Form Program for all members.
What Is the Medical Information Form Program?
Members who choose to participate will complete a short medical information form that includes:
Allergies
Medical conditions
Current medications
Emergency contact details
The club will provide a complementary:
A name‑tag insert / form holder for your medical form
A ring attachment so the badge can hang securely on your pickleball bag
This keeps your information easily accessible to first responders or fellow players if urgent care is needed.
Why This Matters?
In an emergency, every second counts. Having essential medical information available can help responders:
Avoid medications or treatments that could cause harm
Understand underlying conditions
Contact your family quickly
Provide more accurate and timely care
This initiative is completely voluntary, but strongly encouraged. It’s a simple step that can make a meaningful difference for you and for our community.
How to Participate?

Request form holders by emailing volunteers@wkpc.club. The form holder can be picked up at the Mt. Boucherie courts.
